Digital Signature Disclosure and Consent

Note: The application references and other requested items throughout the hiring process are completed through our secure portal using your digital signature.  By continuing to utilize this system, you consent to the following: 


1. You understand that by clicking "submit" or uploading a form with your typed signature, this signature is as valid and binding as though it were complete on pen and paper. 

2. This consent applies not only to the documents on this page, but any others that are completed and returned to Pro Health Care Services using our website and / or online portal. 

3. You may revoke this consent at any time be contacting the Human Resources Department at 614-856-9111 ext. 10. This does not affect the validity of any documents you may have signed / submitted prior to this notification. 

4. Should you not wish to complete or submit any of the required information or documentation using a digital signature, you must contact the Human Resources Department to make alternative arrangements in order to proceed. (Information will then need to be completed or submitted in person at the office.) 

5. You may obtain a copy of any signed / submitted document of yours for your personal records by requesting a digital or printed copy from the Human Resources Department.
