Annual tuberculosis Screening

Annual Tb Screening

The Ohio Department of Health and the CDC have recently updated their regulations on requirements for TB testing for healthcare workers where an annual skin test is no longer a requirement or considered best practice.

In an effort to do our part to ensure the safety of all PHCS clients and Team Members, we perform an annual screening for potential signs or symptoms of active TB (Tuberculosis).

The questions below are only suggestive indicators of infection and are not a diagnosis.

This is required annually for all health care personnel.

How to FULFILL requirement?

Complete the short form below with your answers to each question.

If your response is "Yes" to any question - you may be contacted by a PHCS Team Member for additional follow up. There is an additional space for comments at the end should you wish to add further explanation to your responses.
